Schedule: Every Weekend (On-Call) + 3 Rotating Weekdays

We are seeking a reliable and experienced Home Care On-Call Coordinator to join our team. Key Responsibilities:

• Serve as the primary on-call contact every weekend and three weekdays (rotating)

• Answer incoming on-call phone calls and respond to client and caregiver needs in real-time

• Coordinate and fill last-minute shift call-offs efficiently

• Provide hands-on care and cover client shifts when coverage cannot be arranged

• Utilize home care equipment such as Hoyer lifts, Sit-to-Stand lifts, etc.

• Maintain accurate documentation and communication logs

• Collaborate with office staff to ensure continuity of care and team support

Requirements:

• Minimum 1 year of experience in home care or related healthcare field

• Proficient in using mechanical lifts (Hoyer, Sit-to-Stand, etc.)

• Strong time management and organizational skills

• Excellent communication and problem-solving abilities

• Ability to work independently, especially during on-call hours

• Reliable transportation and valid driver’s license

• Flexibility to work in both office and field settings

Compensation:
This is an hourly position. Pay rate will be based on experience and qualifications.
